Page one of the loan application contains information on what type of loan you will be receiving, where the home is located, and your information at the bottom. Please sign at the top of this page, and initial on the bottom of the page.
Page two of the application contains employment history and income information for your current employer, as well as any alternate incomes you have. Please initial on the bottom of this page.
Page three discusses your current assets (bank accounts, 401k, stocks, etc.) and your liabilities (credit accounts you owe money to), and any real estate you own. Please initial at the bottom of this page.
Page four has a small list of details for the transaction, declarations (yes / no questions about your history) and a government monitoring section listed at the bottom, verifying your ethnicity, race, and sec. Please sign in the acknowledgement and agreement section in the middle of the page.

Under line 8 "Previous VA Loans", input any prior VA Home Loans that you have, even if you don't know all of the information. If you have no prior VA loans, in line 1, please input N/A. The top of page 3 explains energy efficient mortgage guidelines and requires that you check (in the middle of the page, right above "Interest Rate and Discount Statement") if you intend to pursue a VA-EEM or do not intend to pursue of VA-EEM. Please sign and date the bottom of page 3.

Please fill out the entire page, including information about your place of employment, previous employer (if you have been at current job less than 24 months), your primary bank, and other banks. If you are currently renting a home, please fill out the bottom section about rental history.

This is an extension of the full loan application, only used for FHA / VA home loans. This allows the United States Government to guarantee / insure your home loan. You are only required to fill out section 20 on page one.
On page two, you are required to sign once in the first third, and once on the bottom. No signatures are required on page three, although a final signature is required on page four.
